Mortgage Application Checklist |
1. Residence History
|
All applicants must provide the street address for every residence you have lived in during the past two years.
If you are currently renting, please bring:
- Landlord’s name
- Landlord's address
- Landlord's phone number
If you own your current residence, please bring:
- Mortgage lender's name
- Mortgage account number

2. Employment History
|
Provide the following information for every employer you’ve worked for during the past two years:
- Employer's name
- Employer's address
- Employer's phone number
|
3. Income Information
|
All applicants will need to provide the following information:
- Gross monthly salary or other income
- Year-to-date pay stub
- W2s for the past two years
- Signed tax returns if you worked on commission
- Signed tax returns if you wish to use interest, dividend, or bonus income to qualify for your loan
If you’re self-employed, please bring:
- Signed tax returns for the past two years
- Current Profit & Loss Statement
- Current Balance Sheet
If you are not currently employed, please bring:
- Proof of other sources of income*

4. Deposit, Retirement, and Equities Account Information
|
For all Checking, Savings, IRA, Retirement, 401(k), Stock, and Mutual Fund accounts, we will need:
- Name and address of the financial institution holding the account
- Account number
- Account balance
- Statements for the past two months
|
5. Debt Information |
Please bring the following information on all of your outstanding loans and credit cards:
- Name and address of lender
- Account number
- Balance
- Monthly payment
|
6. Real Estate Information
|
For all real estate you own, please bring:
- Address of the property
- Market value
- Loan information
- Copies of leases on any rental property you own
|
7. Personal Property Information
|
Provide the estimated value of your personal property including:
- Furniture
- Vehicles, including make and model numbers
- Boats
- Campers
|
8. Sales Contract
|
Please bring the completed and signed sales contract for the property you are purchasing. |
9. Fees
|
Be prepared to make a payment for:
- Property appraisal
- Credit report
- Flood certification
|
10. Identification
|
Have with you one of the following forms of government-issued photo ID:
- Driver’s license
- State-issued non-drivers ID
- US military ID
|
11. VA Loan Applicants |
If you’re applying for a VA Loan, please bring:
- Military discharge papers (Form DD214)
- Certificate of Eligibility
|
*Alimony, child support or separate maintenance income need not be revealed if you do not choose to have it considered for repaying the loan. |
