Accidental Death & Dismemberment Insurance, Common Carrier Insurance, Dependent Child Coverage, and Dependent Grandchild Coverage — not available to Maryland residents.
Special Program Notes: Accidental Death Insurance is administered by Affinion Benefits Group and underwritten by Hartford Life Insurance Company. A portion of the monthly membership dues, if applicable, will be used to pay the insurance premium to the plan administrator. As a member, you'll automatically receive new benefits as they are added to your program. Coverage is divided equally among co-owners of joint accounts. If more than one benefit is payable, only one, the largest will be paid in lieu of all coverage available. The 24-hour Accidental Death and Dismemberment Insurance benefit reduces by half at age 70. The Dismemberment Coverage is paid as a percentage of the 24-hour coverage. Identity Theft Insurance is offered through the insurance company named on the Benefit Summary included in the membership kit. Please refer to the Summary for complete details of coverage, limitations and exclusions. Identity Theft coverage not available for residents of New York and could become unavailable in other jurisdictions.
IMPORTANT NOTICE: YOUR LIABILITY FOR FRAUDULENT CHARGES UNDER FEDERAL LAW:
For credit cards: If the card issuer has notified you of your maximum potential liability, has provided a means for you to notify the card issuer of credit card loss or theft, and if the credit card contains a way of identifying the cardholder or authorized user, then your liability for fraudulent charges made to your account before the card issuer is notified is up to $50 for each card.
For debit and cash-machine cards: Your liability for fraudulent use of your cards is up to $50 if you notify the card issuer of the loss or theft of the card within two business days after learning of the loss or theft of the card. After that, your liability is up to $500 for each card. Or, if you do not notify the card issuer within 60 days of receipt of a monthly statement showing unauthorized transfers, then your liability for each card is up to the total amount of unauthorized transfers that occurred after the 60-day period and before notice, provided that the card issuer establishes that the unauthorized transfers would not have occurred if you had notified the card issuer within the 60-day period. You may also be liable for up to $50 or $500 for each card as described above, as applicable.
